Chest Seal Types Explained: Vented vs. Occlusive
Penetrating chest trauma — a gunshot, stab wound, or other puncture to the chest — carries a specific risk that most other wounds don't: air entering the chest cavity through the wound itself. Left untreated, this can lead to a tension pneumothorax, where trapped air puts dangerous pressure on the lungs and heart. Chest seals exist to manage that risk, and the type you use matters.
The Core Problem Chest Seals Solve
A normal bandage doesn't create an airtight barrier, which for most wounds is fine — but for a chest wound, air can be pulled into the chest cavity with every breath. A chest seal creates an airtight (or nearly airtight) barrier over the wound to stop that from happening.
Occlusive Chest Seals
An occlusive seal creates a completely sealed barrier over the wound with no way for air to escape. This sounds like the obvious solution, but it has a real drawback: if air is already trapped in the chest cavity, a fully sealed dressing traps it there too, which can make a tension pneumothorax worse rather than better.
Vented Chest Seals
A vented chest seal, like the HyFin Vent, uses a one-way valve design. Air can escape out through the valve when the patient exhales, but the valve closes to prevent new air from being pulled back in. This is why vented seals are generally the preferred choice in current guidelines — they get the sealing benefit without the trapped-air risk.
Which One Should You Carry
For most kits, a vented chest seal is the safer default choice, which is why current tactical and EMS guidance generally favors them. Some kits include both an occlusive and vented seal, since a penetrating chest wound often has both an entry and exit point, and having two seals on hand covers that scenario.
How They're Used
- Locate and expose the wound — check the back as well as the front for an exit wound.
- Wipe the area as dry as possible so the adhesive seals properly.
- Apply the seal directly over the wound, pressing firmly around the edges to ensure a full seal.
- Monitor breathing closely afterward — chest trauma can deteriorate quickly and needs professional care as soon as possible.
